Healthcare for Homeless People

In the Autumn the “Babelas Wednesday” cycling club will be riding from Lands End to John O'Groats to raise money for the Healthcare for Homeless People project.  The riders are Dr Nigel Hewett who leads the UCLH Homeless ward round, with friends Alistair Reid, Phil Lord, Tim Shaw and Mel Berry, with support vehicle driven by Gareth Reid.

Homelessness destroys lives, families and fractures communities.  It breeds despair and anger, and contributes in a large way to the inequalities in our society.

 

The life expectancy of someone sleeping rough is estimated to be 40.2 years. 

 

We are seeking funds to develop a UCLH homeless healthcare service, which responds to an ‘invisible’ patient group within UCLH who have a high impact on our health service, while, at the same time, lack a voice and any sort of representation.

 

The vision

Our vision is of a service ‘without walls’; a service that incorporates health care, cuts across artificial boundaries and is tailored to individual need.  A service based on compassion, accountability and the restoration of dignity, responsibility and hope.  There are four key components:

 

Homeless ward rounds in central London hospitals (being pioneered now at UCLH), to pull together the disparate efforts of social workers, physiotherapists, drug and alcohol workers, psychiatrists, housing representatives, primary care teams, and discharge sisters; and coordinate care for the complex needs of homeless people, both in hospital and on discharge.

 

A model of perfect partnership reaching out from homeless ward rounds to forge long-term links with homeless people following discharge, to help them rebuild their lives and ensure their needs are being met in the community. Teams will be led by Homeless Health Practitioners - trained experts in homelessness and homeless health care – and include: a GP; a social worker; a psychologist; a substance misuse worker; outreach workers (care navigators); as well as part-time input from a psychiatrist and infectious diseases physician. 

 

Care navigators; people who have experience of homelessness who can offer emotional support and assertive outreach for those most in need, and link them up with services.

 

A sanctuary; a pan-London intermediate care facility offering intensive, holistic, health care and emotional support for those with the most complex needs, networked to other hostels that provide differing levels of primary care.  This will also serve as a base for our teams and a beacon for best practice in services for homeless people.
