What is the Alexandra Centre?
A multidisciplinary specialist centre, operated by the Delirium and Dementia Outreach Team (DDOT) to support patients who are at risk of cognitive difficulties (problems of memory and understanding) such as Dementia and Delirium. The Centre also has a role to support carers and staff within the trust by providing information, education and specialist input to the wards. People with Dementia can be admitted to hospital due to many reasons, for example: infections, heart failure, falls or social issues.
These patients have complex needs requiring specialist input. There is good evidence that therapeutic activity and good basic care improves outcomes for this group of patients. The Alexandra Centre aims to act as a centre of excellence to provide this care and raise standards across the Trust by using an in reach/outreach approach.
We will be working alongside Daniel Taylor from BGL to gain matched funding.
This service is run by DDOT and comprises:
• Therapeutic activities sessions in the Alexandra Centre.
• One to one support and activities for patients on the wards.
• Education both within the Alexandra Centre and on the wards for staff and carers (Dementia Friends).
• Drop in service where partnership organisations provide information.
