I'm hoping to raise £750 for this amazing service. My family and I are truly grateful for the Acute Care@Home team with the Southern Health and Social Care Trust for all they have done for our Granda Billy Muldrew over the past year.

Without this amazing team we wouldn't have been able to keep Granda at home, where he wanted to be, and he would have been admitted to hospital.

Acute Care@ Home has provided the best treatment and attention our Granda could possibly have received and have made his quality of life so much better as well as allowing us to spend this precious time with him.
