My amazing dad rode his bike daily for over 50 years and he taught me how to ride. I still love riding and taking his grandson's out on their bikes which they really enjoy.
Saturday 23rd June 2018 he was out riding just a few miles from our family home on a lovely sunny day in Portsmouth and something went wrong and he wobbled of the shared pedestrian / cycle pavement out onto the road and a single decker bus with passengers on going 25mph had no chance to stop or avoid him and he died shortly after:

A great idea we have heard about since is for segregated cycle lanes where there is a safety barrier so even if a cyclist has a wobble or a puncture or collides with something they might hurt themselves a bit falling into the barrier but at least they wouldn't suddenly fall into a busy road with moving vehicles.
