Our Story

The Tim McCormick Transitional Care Center is a 40-bed rehabilitation community dedicated to helping patients regain strength, independence, and confidence before returning home.

Every day, individuals arrive facing challenges after illness, surgery, injury, or hospitalization. Through expert rehabilitation, compassionate nursing care, and unwavering encouragement, we help patients achieve goals they once thought impossible—taking those first steps, regaining independence, and ultimately returning home to the people they love.

Many former patients and families tell us they wish there was a way to thank the team that helped them through one of the most difficult times in their lives. Because staff cannot accept personal gifts, the Tim McCormick Transitional Care Center Fund provides a meaningful way to express gratitude while supporting future patients and caregivers.
