Since its inception in 2015, Transitions Kids continues to offer community-based palliative care, hospice and bereavement services to children and families impacted by illness and loss. The care that we provide is tailored to meet the unique needs of each family on their sacred journey of love, exasperation, triumph and grief. Today, Transitions Kids remains the only home hospice program for children in the Triangle. In the first year of operation, Transitions Kids cared for just under 50 individual patients. In 2022, Transitions Kids served 233 children and their families through hospice, palliative care, and bereavement services.
In 2021 Transitions Kids took over pediatric grief services, creating a fully comprehensive pediatric program for any child or family impacted by serious illness or loss. In just one year, Transitions Kids went from providing bereavement services from 57 kids or families to 110. One of the grief offerings for children that Transitions Kids provides is Camp BRIGHT, a one-day camp for youth ages 5-14 who are impacted by the death of a significant person. Therapeutic techniques are creatively infused by trained bereavement specialists to create a safe, fun, loving and educational atmosphere, where families can continue their journey toward healing. Camp is offered at no charge to families.
Your donation helps us provide whatever a family needs as they face severe illness or end of life. For example:
$500: one week of personal care and attention from a hospice aide
$200: one day of overnight respite care for caregivers
$125: one home visit from a hospice nurse in the middle of the night
$75: one grief counseling session provided free of charge
