Hennepin Healthcare Hospice provides care to hundreds of patients each year. These patients are from all walks of life. Some patients have families surrounding them with love and support, and others come to us alone. Some have financial resources, and others may be homeless. Some are ready to face their final days, and others need more time to process and accept that they are nearing the end of their life. Our patient-centered care model means we work to individualize the care for each patient and their circumstance.
The Hospice care team includes a physician, registered nurse care coordinator, hospice home health aide, social worker, chaplain, and volunteers who provide companionship. To enhance our hospice care, we offer complementary therapies, which include pet therapy, music therapy, and massage therapy. These valued services help to alleviate physical, emotional, and social suffering and add to our patients' quality of life.
