Well Child do amazing work, ensuring that children with complex care receive the right discharge planning and support for their return home or a provider nearer to where they live.

The well child nurse at Addenbrookes is the link person who co-ordinates complex discharge either to a child's local hospital, rehab or hospice.  They link in social care, community teams and provide ongoing support for the families and carers.  This is an open access service.
