Celebrate the life of a loved one who is no longer here but will always have a special place in your heart, a special anniversary or the birth of a child by lighting a light in one of our trees.

**Please note names need to be received by the 18th November**

The trees are lit at a short service at The Countess of Chester Hospital on the 2nd December. For more details please contact emma.boylan1@nhs.net 

If you choose to donate via Just Giving - the name you are celebrating will be on Just Giving and in the book of celebration, but we will not be able to send a card.

The donations are used to support Bereavement Care at The Countess of Chester and Ellesmere Port Hospital.

ALL online donations will be named in the 'Book of Honour'
