The POD Team predominately work with patients who have dementia, delirium, are at risk at falls, or who are at risk of harming themselves or others. The aim is to foster a positive therapeutic relationship and monitor the patients well-being

We raise funds for all wards and departments at Calderdale and Huddersfield NHS Foundation Trust, enhancing the experience of patients, their families and staff, over and above what NHS funding alone can deliver. Together we can make a real difference to local people when they need it most.

Story

The Prevention of Delirium Team project began in August 2016 and was created as a care approach to support people at risk of developing delirium in hospital as well as to support those living with dementia. The project took two years to develop and has continued evolving into the team we have today. All of the resources used to provide patient engagement are purchased through charitable funds from nail polishes to Reminiscence Interactive Therapy Activity units (RITA).
