We are raising funds to help offer extra support to patients who are on end of life care and to their loved ones
The time after someone has died can be incredibly difficult for loved ones, therefore it’s essential that they are supported as much as possible prior to and after their loved ones death.
Care and memory packs will allow us to create a person centred care approach, emphasizing personhood after death, which will support the mental wellbeing of relatives.
Family members will keep these memories with them forever, which can have a big impact on the rest of their lives. We would like to offer comfort to as many people as possible and your support can help us to do this.
With the support of the East Cheshire Charity and donations from the public the End of Life Care team at East Cheshire NHS Trust will have access to resources that can be offered to patients and their loved ones at this sad and difficult time. These resources will help to provide individualised, unique care to the patients and their loved ones and help to make positive lasting memories.
