1. REGISTER FOR MEETING at https://www.surveymonkey.com/r/2018PSM 2. MEETING FEES Paid before September 1, 2018: £70 per person 15 years or over; £30 per child ages 2-15. Paid after September 1, 2018, the fee increases to £90 per person 15 years or over, £45 per child ages 2-15.
Meeting fees are waived for a PC patient and one family member attending the PC Patient Support Meeting for the first time. PC Project pays 70% of the meeting costs which include the meeting room, equipment rental fees and food including Friday dinner, Saturday lunch and dinner, and Sunday lunch as well as break food. Breakfast is included with hotel room reservations.
