Alan had Vascular Dementia which meant he was cared for at home by family until the end with the support of three teams. The carers who were relentless in their support for our family. The District Nurses who aided his medical needs at home and bought expertise and excellence with them and the Thames Hospice Palliative Team they were available for advice and help all times of the day and night and bought with them caring and brilliant advice with reassurance and made an exceptionally difficult time easier meaning Alan died at home with family which is what he wished.
