Story
My Name is Renee Ward and I am the lead nurse for Frailty Interface at Ipswich Hospital.
My role is twofold, primarily I work in the emergency department where I am the central point for the management and safe discharge of the older person living with frailty and secondly, I work as a practitioner at the hospitals The Frailty Assessment Base (FAB). FAB is an admission prevention service based at Ipswich Hospital. This service is for the older person living with frailty who are experiencing a crisis situation i.e. change in health needs, carer breakdown or risk to safety, for which without support would result in an acute hospital admission or A&E attendance.
Within both roles my aim is to support our older population in maintaining their dignity, health and wellbeing needs and independence within their own home setting.
I am aiming to complete the Ipswich Hospital Charity event the Yorkshire Three Peaks walking challenge.
This challenge is a 24-mile trek over the peaks of Pen-y-ghent,
Whernside and Ingleborough in a one-day period, Saturday 4th of August.
The funds I raise will be used to buy equipment that will enable staff of the Frailty Assessment Base to perform tests that support same day diagnosis, interventions and care and will also support enhancing the emergency care environment for the older person.
