The Swan End of Life Care Model was introduced at The Pennine Acute Hospitals NHS Trust and created at Salford Royal Foundation Trust in 2012. Swan offers dedicated support to patients in the last days of life and to their families into bereavement and beyond.

End of life care is a difficult period for patients and relatives and there are many choices available to them, for example the decision to sign the organ donor register and whether they would prefer to die at home. The SWAN Charitable Fund was set up to support training and enable staff to fund all of the bespoke equipment to help create moments that will turn into memories that will last forever.
