Home From Hospital Care supports Birmingham residents who are leaving hospital and recovering at home. They provide a discharge support service to make the discharge process a quicker and more positive experience. They also provide an aftercare support service which helps people after they have left hospital and whilst they recover at home. A period of illness and a hospital admission can send shockwaves through a person's life. The process of getting home and recovering can be a complicated and difficult time. Being medically fit to leave hospital is one thing, but 'getting better' is much more than just a physical process. Home For Hospital Care provides a range of support services to help limit the negative physical, emotional and financial impact of a period of illness and a hospital admission. We work to reduce worry and anxiety, loneliness and isolation, maximise recovery and help people to stay safe and well at home for as long as possible.
