The hospice does an amazing job at the most difficult of times. The care and environment they provided not just for mum, but for her family during her end of life care. It made a huge difference to us knowing how well looked after she was and gave us a space to say goodbye.

We have a team of dedicated community nurse specialists who deliver care to patients with life-limiting illnesses in their own homes across the service area. They cover approximately 26,000 miles across North Somerset every year caring for patients at home.Your Hospice Community Nurse Specialist can:Offer support to you in your own home (which could include care homes) throughout your illness so you can live independently.Work with and advise your GP, district nurse, other healthcare and social care workers to provide you with pain and symptom relief, emotional and spiritual support, and help with accessing financial or other support as needed.Advise you on all of the hospices services and liaise with other teams to provide you with the support your nurse, your family and yourself feel is needed.Give extensive expertise in palliative care and will share that knowledge to help your support network (including family and friends) look after you.
