Over the past year I have referred a number of patients to Age UKs home from hospital service which has been a great relief for me to know there is extra support.

In particular I had an individual readmitted a few months later who I referred to for loneliness. He expressed how amazing this service was and he no longer feels isolated. This could be any of us at one time in our lives and having services like this within the community is very special.
