Support this ambitious £3 million build project for a modern re-ablement facility (RISE Centre) for medically fit patients on discharge from London hospitals, with full help to enable them to return home. The new RISE Centre will be located on the three floors above the current building at the Platt in Felsham Road, Putney, providing:

• 12 ensuite bedrooms

• tea and coffee facilities

• a support worker

• access to local services
