This year, ADSS is celebrating 35 years of supporting people living with dementia, their families and carers.

To mark this incredible milestone, we’re taking on a 30km walk, alongside 5 fun activities, to raise money and awareness for the vital work ADSS does across our communities.

ADSS provides personalised support, care and activities that help people affected by dementia to stay connected, independent and supported throughout their journey. From Dementia Coordinators and support groups to day services and wellbeing activities, ADSS helps make sure that nobody has to face dementia alone.

ADSS also provides a specialist Hospital Dementia Coordinator Service, supporting people living with dementia and their families during what can be one of the most difficult and unsettling experiences — a hospital admission.

Our Hospital Dementia Coordinators work closely with patients, families and hospital teams to ensure that a person's needs, wishes, routines and communication preferences are understood. They provide reassurance, guidance and advocacy, help avoid unnecessary hospital admissions where appropriate, and support safe, well-planned discharge back home.

Our support continues beyond the hospital through our Enablement Service, helping people regain confidence and independence following discharge, re-establish everyday routines and remain independent in the place they know best — their own home.

Together, these services help provide joined-up support from hospital to home, making a real difference to the dementia journey and experience.

We’re taking on this challenge to celebrate the amazing work ADSS has done over the past 35 years — and to help support the next 35.
