The Swan Fund, SaTH Charity was set up by myself in 2015 when my lovely Dad died at the hospital on AMU in July 2015, it would have been his birthday on the 20th of June, so me (Swan EOLC Lead Nurse) and Heather our (Staff Bereavement Support Trained Volunteer) & Friend will walk across Wales 🏴󠁧󠁢󠁷󠁬󠁳󠁿 from Staylittle to Montgomery, 30 miles. The money raised will be spent on Swan Model of Care resources to support good and excellent last hours and days care at The Shrewsbury and Telford Hospital NHS Trust (SaTH) Staff gift swan folders and bereavement bags with helpful items at the hardest of times. The swan folders include a letter, information leaflets, tissues, pen and paper, the swan bereavement bag includes a letter, tissues, bereavement booklet and the good grief trust card for signposting for further bereavement support, to support the people important to the person who is dying or who has died, at the hardest of times.

We also develop swan rooms to improve the environment, they are freshly painted in calms colours and have a skylight, reclining chair, picture, moodlight and more.

Swan Suites to have important, open and honest conversations with patients and people important to the patient, we have these in renal, respiratory and stroke units.
