The Palliative Care Team at Bradford Teaching Hospitals works closely with Bradford Hospitals Charity to thoughtfully select and provide Memory Making Packs for patients and their families.

With family at the heart of everything they do, the Palliative Care Team offer these resources—funded by Bradford Hospitals Charity, to help capture precious moments during end-of-life care.

Each pack is carefully tailored to meet the individual needs and wishes of the recipient. It may include items such as a packet of seeds, a hand-print, a knitted heart, or a key ring. Every Memory Making Pack is unique.
