Integrated care is designed to ensure a client has all the services they need in place, to build resilience and reach their own individual level of success. We focus on 4 main supports, health services, developmental, economic and social/emotional.

We utilize our Integrated Care Tool to get a complete picture of a person’s current circumstances. This helps guide us to work alongside our clients to create care plans that are tailored to their needs and goals.
