In his Intentions and Wishes document Tim wrote "I wish to die at home".  This wish could not have been fulfilled if it were not for the incredible Community Palliative Care Team.  Without exception every member of the Team who visited Tim at home showed warmth and compassion towards him and his family.

Again, quoting Tim's words "I want to say a big thank you to the professionals who have treated me with great kindness as well as skill".
