As part of an integrated discharge team we support and care for people with life limiting conditions.  We  have been involved in supporting people and families who have been affected by Motor Neuron disease and would like to fundraise to make much needed funds to make a difference to those affected by such a debilitating disease. As a team we have set ourselves a challenge to walk 50k and would be so grateful for your support and sponsership.
