We are a team of colleagues working on Healthcare and Integration projects in Coventry, Gloucestershire, and Leeds. Our day-to-day job is all about helping NHS trusts and local councils to work together to provide more joined-up care for elderly patients. We see on a daily basis how much of a difference having the right care at the right time can make to patients with dementia and their families, and are keen to do more to help.
